Appointment cancellation
If you are unable to keep an agreed appointment, please cancel it at least 24 hours in advance.

Transfer slip
As a diabetological practice, we need a referral slip from your family doctor or gynecologist, as otherwise we cannot invoice the statutory health insurance for any services.
If you are registered with us in the DMP, you are not required to provide a transfer slip.
