unable to keep an agreed appointment, please cancel it at least 24 hours in advance.<\/p><p><strong>Transfer slip<\/strong><br \/>As a diabetological practice, we need a referral slip from your family doctor or gynecologist, as otherwise we cannot invoice the statutory health insurance for any services.<br \/>If you are registered with us in the DMP, you are not required to provide a transfer
